If this checkbox is activated, the import-picture dialogue is displayed whenever you open a new design. But if you don’t need a background picture as a rule, because you always create monograms for example, you may switch off this checkbox. 

And if you want to import a picture nevertheless, you can call this window with the import picture entry in the file menu.
